However, in compliance with the Family Educational Rights and Privacy Act (FERPA), the University is prohibited from providing certain information from your student records to a third party, such as information on grades, billing, tuition and fees, financial aid and other student record information. This restriction applies, but is not limited, to your parents, your spouse/partner, or a sponsor.
FERPA does allow the University to release information from your education records to parents/guardians who claim you as a dependent on their most recent federal Income Tax return. To demonstrate that this provision is applicable, your parents/guardians must submit documentation to the Office of the University Registrar. Additionally, FERPA provides that you may, at your discretion, grant the University permission to release information about your student records to a third party, such as a parent or guardian, by submitting a completed Consent for Disclosure of Information from Education Records Form (Consent Form). On this form you can designate anyone as a third party to whom the University can release information from your student records, including but not limited to grade and transcript information, class registration details, participation or progress in classes, and billing or financial aid information. Please note that this consent allows any school official, not just an advisor or instructor, to provide information from your student records but only to an individual specifically named on your consent form. This consent also allows these listed third party designees to request certain records from the Office of the University Registrar on your behalf, such as a transcript, grade point average, class rank or other non-directory information. You may revoke your consent at any time either by giving written notice of your revocation to the Office of the University Registrar or by submitting an updated Consent Form to the Office of the University Registrar; or students in the College of Arts and Sciences can submit an updated Consent form.
You may use the following Consent for Disclosure of Information from Education Records to designate the people you wish to authorize to obtain information about you from the University.
Important : The Office of the University of Registrar’s policy is to overwrite any UNC Chapel Hill Student FERPA Release Forms submitted previously.
By typing my ONYEN and electronically submitting this form, I acknowledge and understand that this consent will remain in effect until such time as I revoke it by giving written notice of my revocation to the Office of the University Registrar or by submitting an updated Consent Form to the Office of the University Registrar or to the College of Arts and Sciences (for students in the College). I understand that any such revocation will be effective for disclosures that have not yet occurred at the time the University receives my notice.
