Complete and submit the UNC-CH Veterans Educational Benefits Enrollment Data Form from your UNC email address.
Important: Please submit only 1 semester per form.
*** VA will not pay benefits for credits excluded from GPA/hours, and/or courses not included in your degree program.
*** If your program of study does not follow the standard semester dates, you must get documentation including specific dates (1st day of class until last final exam) from your appropriate department official.
Please remember to notify the VA certifying official of any changes in hours.
Note: For Ch. 33 Post-9/11 students only, the VA will pay this fee if you do not have your own health insurance and cannot waive out of the University’s plan.

By submitting this form, I agree to advise the UNC-CH Veterans Services Officer of any changes to the above information and understand that failure to report changes to my enrollment may cause an overpayment for which I would be responsible for repayment to the V.A. If I fail to report changes to my enrollment status, I understand that this office reserves the right to process subsequent certifications by semester and/or after the semester is completed. I acknowledge that the UNC-CH Veterans Services Office may release non-directory information to the Veterans Administration as needed to comply with V.A./Government for the VA to process the claim.
